Player Insurance

The below details the procedure for making an insurance claim by members of the club for injuries sustained while playing or training for Swords RFC.

Procedures for making claims in respect of medical expenses

1. Complete Personal Accident section of claim form (Click here to download a form).
2. Have the Medical Certificate section of the claim form completed by the appropriate Medical Practitioner.
3. Obtain and submit receipts in respect of treatments.
4. Return completed forms along with receipts to Ben Traynor.

Pleas note that there is a maximum limit of €625 per injury with an an excess of €100 per claim.
